John Jenkins Nomination Form
Guidelines for completing the John Jenkins Award nomination form:
- Nomination must be supported by three voting members of CSA Group
- Employees and members of the CSA Group Board of Directors are not eligible for the John Jenkins award
- All four criteria sections on nomination form must be met
- John Jenkins candidates must have received an Award of Merit or a Personal Award of Distinction in the past
